Telephone Consultation

This is appropriate only if you are currently under the care of a physician and you wish to have a "second opinion" by Dr. Theo about your osteoarthritis. This is not appropriate if you need a diagnosis of what's causing your pain or problem. A confirmed diagnosis must be pre-established.

In accordance with insurance requirements and the Arizona Board of Medical Examiners, Dr. Theo must record the conversation for medical record keeping.

Payment is made in advance of the appointment. Checks or money orders can be mailed to Jason Theodosakis, MD at this address: (6890 E. Sunrise #120-383 Tucson, AZ 85750). Credit cards can be used for payments only through the payment systems of www.Paypal.com by e-mailing money to

Payment amount is $395 for up to 60 minutes. Additional time is billed at $225 for each 30 minute increment.

To setup a phone consult, we simply schedule a time by e-mail for you to call Dr. Theo's office. Any x-ray reports, lab reports, MRI reports or doctor notes can be faxed or e-mailed to Dr. Theo prior to the appointment at the following:

Due to time constraints, you cannot call Dr. Theo to ask questions about the details of appointments, only e-mail queries will be entertained.

A receipt for the phone consult can be mailed or faxed to you but Dr. Theo does not accept or bill insurance. In general, insurance will not pay for telephone consultations, no matter what type of documentation you provide. The appointment cost may be applied against an insurance deductible or you may be able to use money from your medical savings account (Flexible Spending Account or Health Savings Account) to pay for the cost of the appointment.

All of these conditions are required if you wish to proceed with an appointment.
